Too many patients with chronic wounds never get the specialist care they need not because it doesn't exist, but because getting there isn't realistic. Transportation, mobility limitations, and the demands of daily life get in the way. Optimal Wound Care exists to close that gap.

We bring advanced wound care directly to patients in their homes and in the facilities where they live. Our mission is simple: no patient should have to put life on hold just to heal.

Founded by Virginie, a Nurse Practitioner and chronic wound specialist, Optimal Wound Care provides mobile, specialist-level wound care across Pennsylvania and Maryland. Every visit comes fully equipped with supplies, and providers are scheduled within 24 hours because wounds don't wait.

We work closely with the care teams already supporting these patients: home health agencies, assisted living facilities, rehab centers, and case managers - stepping in as specialist backup when wounds become complex or stop progressing.

We partner with home health agencies to manage chronic, non-healing wounds that require specialist-level care including debridement and advanced wound oversight. Our team works alongside your nurses to stabilize difficult wounds and improve patient outcomes, while your agency remains in full control of the care plan.

HOME HEALTH NURSES

When wounds stop progressing or become too complex to manage alone, we step in as your clinical backup. We provide specialist wound care support so your nurses can focus on what they do best  with the confidence that complex cases are in good hands.

ASSISTED LIVING FACILITIES

We bring specialist wound care directly to your residents, right where they live. From chronic pressure ulcers to diabetic wounds, we help residents heal faster, reduce unnecessary hospital transfers, and ease the burden on your care staff.

IN-PATIENT REHAB CENTERS

Discharge shouldn't mean a gap in wound care. We specialize in seamless post-discharge wound management for patients returning home or transitioning to a facility reducing the risk of complications and keeping readmissions low.

HOSPITAL CASE MANAGERS

When patients are discharged with wounds that still need specialist attention, we make sure the care continues. We provide reliable post-discharge wound management so case managers can discharge with confidence, knowing their patients won't fall through the cracks.

Virginie Tokha is a board-certified Family Nurse Practitioner specializing in chronic and complex wound management. She provides advanced wound care services to patients in home health, assisted living, rehabilitation, and post-acute care settings throughout Pennsylvania and Maryland.

Many patients experience wounds that do not heal properly due to underlying medical conditions, limited mobility, or interruptions in continuity of care. Virginie’s focus is to bring specialized wound care directly to where patients live, allowing them to receive expert treatment without the challenges of traveling to outpatient clinics.

Through Optimal Wound Care, she works closely with home health agencies, physicians, case managers, and senior care facilities to ensure patients receive timely evaluations, evidence-based treatment, and consistent follow-up care.

Her goal is to accelerate healing, prevent complications, and reduce avoidable hospitalizations while supporting the broader healthcare team involved in each patient’s care.

Virginie is deeply committed to improving patient outcomes while supporting healthcare partners in managing complex wound care needs.
