Referral Information

Optimal Wound Care partners with healthcare providers, care teams, and facilities to help patients receive timely specialist wound care in the comfort of their home or residence.

Our mobile wound care team manages chronic and complex wounds while coordinating closely with the patient’s existing care team to support healing and continuity of care.

We aim to make the referral process simple and responsive. In most cases, patients can be evaluated within 24 hours of referral, helping ensure timely treatment and better outcomes.

Step 1 – Submit the Referral

Step 2 – Patient Review

Our clinical team reviews the referral and coordinates scheduling with the patient or facility.

Step 3 – Patient Evaluation

A wound care provider evaluates the patient, typically within 24 hours of referral.

Step 4 – Ongoing Care & Coordination

We provide ongoing wound management and communicate with the referring provider and care team to support continuity of care.

A referral may be appropriate when a patient has a wound that requires specialized evaluation or treatment. Common situations include:

Wounds that are not progressing as expected
Chronic or non-healing wounds
